Forms Plan/Work & Permits Forms Borough Intake Form The Borough Intake Form is used to submit manual applications to the Borough Office. The form improves tracking of submitted requests and allows the Department to send denial notifications electronically. The form must be submitted with the following applications: PW1 PAA, PW1 Withdrawal, PW2, PW6, and PW7. 7/17 PW1 Plan/Work Approval Application The PW1 is the first document filed with the Department to begin the application process. 4/18 or PD1 Plot Diagram The PD1 form allows the Department to collect plot diagram information in a uniform way for all applications. It will be required prior to approval with all initial filings for New Building and Alteration Enlargement applications.
